Part of the lineup waiting to go into the "Grotto of the Nativity", a small cave beneath the church which they claim was the spot where Jesus was born. At that time, caves were often used as stables, with homes built in front of them, so it's entirely possible this one housed a manger. There were people visiting here from all over the world.
